- Two routine exams, two cleanings, two fluoride treatments, and yearly X-rays, covered in full
- No pre-existing condition exclusions, waiting periods, or deductibles
- 20% off every other treatment when paid in full
- Coverage starting at $345 a year for a single member, with options to add a spouse or dependents

What the Plan Covers
Program disclaimer: The fees outlined in the Complete Smiles Dental Membership Plan are non-refundable. All plan fees are due at time of enrollment. An additional fee may be charged for any missed, cancelled, or broken appointments without 48 hour notice. The plan is non-transferrable by patient. The plan cannot be combined with any other insurance, dental plans, coupons or discount. Plan participants are responsible for scheduling their periodic treatments, and services not utilized at the end of each year's membership period are not carried over to the following year. Discounts apply only to services rendered by Complete Smiles Dental and do not include treatment or services provided elsewhere if you are referred to a specialist. When CareCredit is used, the amount of the discount will be decreased by 10%.
